Bronze and Silver Award Request Form
This form should be completed by troop leaders or Silver Award Consultants to request certificates for Girl Scouts who have completed their Bronze or Silver Award.
Please submit one form per project. If multiple Girl Scouts worked on a project together, you will have the opportunity to include all of their names on one form.
Once the certificates are received, award pins can be purchased at the Girl Scout shop by showing the certificate to the cashier.
