Volunteer Driver Form
Thank you for volunteering to drive Girl Scouts to planned activities and trips. To remain in compliance with our insurance provider, we need to collect some information from adults who will be transporting youth not in their household. Please complete and submit this form at least two weeks prior to the planned activity for which you will be driving. You only need to submit this form once per Girl Scout year (October 1 - September 30) unless your personal information changes.
