Product Program Collections Form
The service unit fall product manager (SUFPM) / service unit cookie manager (SUCM) or troop fall product coordinator (TFPC) / troop cookie coordinator (TCC) will complete the applicable portions of this form to report a troop or individual delinquent in their payments according to official program schedules. The troop will be held accountable for debt unless council receives this completed form and all required attachments. Please remit a complete form for each delinquent individual to council by the dates listed below:
- Fall Product Program: submit form no later than Monday, November 25, 2024
- Girl Scout Cookie Program: submit form no later than Monday, March 3, 2025
