Service Unit Awards Order Form
This form is used by service unit leadership and service unit volunteer recognitions coordinators to request service unit award pins. Explore the Volunteer Awards Guide within Service Unit Resources for detailed descriptions of each award and candidate eligibility criteria.
- All orders must be received at least four weeks prior to the pick-up or mail arrival date.
- Limit one submission per service unit per membership year (October 1 through September 30).
- The awards are provided at no cost to the service unit. Awards can be mailed or picked up at the Kodosky Service Center.
- Before submitting this form, service unit leadership should use Looker to ensure award recipients are registered Girl Scout adults with current, eligible background checks on file.
Limit two service unit awards per volunteer per membership year.
- A volunteer is eligible to receive up to two different service unit awards if the awards are recognizing service for two different volunteer roles such as hosting a service unit event and serving as the service unit treasurer.
- The Service Unit Outstanding Leader Award and Service Unit Outstanding Volunteer Award are given only once to a volunteer in recognition of their outstanding service.
- Consider nominating a volunteer eligible for multiple service unit awards for a council or national award. Submit the Council and National Award Nomination Form by April 1.
